We are dedicated to helping patients achieve their health and wellness goals. For new patient consultations, patients are seen by Dr. Evangelista for an in-depth new patient intake visit. We ask all new patients to please arrive 15 minutes prior to your appointment time to complete your initial paperwork. We recommend bringing any supplements or medications you are taking, as well as any medical records you may have.
Your scheduled consultation is reserved exclusively for you. In the event you should need to cancel or re-schedule your consultation with Dr. Evangelista, please notify us 24-hours in advance to avoid any fees. Same day cancellations and "no shows" will be charged the full amount of the consultation.
A credit card is required to secure all appointments. Financing is also easily available through CareCredit.
At Ajeless Health & Medical Spa, our goal is to provide quality medical care in a timely manner. "No-Shows" and late cancellations inconvenience those individuals who need access to timely medical care.
In order to be respectful of the medical needs of other patients, please be courteous and call Ajeless promptly if you are unable to show up for an appointment. We understand that things come up and it may not be possible for you to keep your scheduled appointment. However, if it is necessary to cancel your scheduled appointment, we ask that you call as soon as you know you will not be able to make it.
Appointments are in high demand and if you give us more than 24 hours notice, your early cancellation will give us a chance to reallocate the appointment slot to someone who is in need of treatment. If you give us less than 24 hours notice (late cancellation), we will likely not have time to fill the empty slot.
A cancellation is considered late when a patient contacts AJELESS to cancel their scheduled appointment with less than 24-hours advance notice and cancellation fees will apply.
A "no-show" is determined when a scheduled patient misses their appointment without contacting Ajeless. A failure to be present at the time of a scheduled appointment without a call canceling the appointment will be recorded in your medical record as a "no-show" and fees will apply.
