NEW PATIENT CONSULTATIONS:

We are dedicated to helping patients achieve their health and wellness goals. For new patient consultations, patients are seen by Dr. Evangelista for an in-depth new patient intake visit. We ask all new patients to please arrive 15 minutes prior to your appointment time to complete your initial paperwork. We recommend bringing any supplements or medications you are taking, as well as any medical records you may have.

Your scheduled consultation is reserved exclusively for you. In the event you should need to cancel or reschedule your consultation with Dr. Evangelista, please notify us 24-hours in advance to avoid any fees. Same day cancellations and "no shows" will be charged the full amount of the consultation.

Cancellation Policy

Appointments require a deposit of 50% of the cost of the scheduled procedure/treatment. Deposits are applied to the full cost of the scheduled procedure/treatment. Deposits are refunded for appointments canceled no later than 48 hours prior to the scheduled date and time of the procedure/treatment. A late cancellation fee totaling 25% of your deposit will be applied to all appointments canceled less than 48 hours before the scheduled procedure/treatment.

The patient agrees to pay the full amount due at the time services are rendered and treatment is received. Patients hereby understand and agree that they are responsible for a cancellation fee and understand that if they cancel any appointment with less than 48 hours prior notice a late cancellation fee will apply.

How to Cancel Your Appointment:

To cancel an appointment please call 248-465-8000, If you do not reach the receptionist you may leave a detailed message on our voicemail. If you would like to reschedule your appointment, please leave your phone number. We will return your call and give you the next available appointment time.
